Is it okay to put the address of the husband (private accommodation Bangkok) or does this have to be a hotel address?  Would the spouse then have to file a residence notification at Chaeng Wattana within 24 hours of arrival if private accommodation?

It should be the address where you are staying. They should use their husbands address. It does not have to be a hotel address.

Chaeng Wattana immigration does not want a TM30 residence notification from individuals. They only enforce it for hotels and etc.
