You can't chose your doctor at  government hospital like Queen Sirikit. They'll send you to one of their orthos.

 

If you want to chose a doctor with expertise in shoulder injuries, and given that I gather you are in Pattaya and likely in no shape to travel (the best shopulder specialist i nthe ocuntry being in Bangkok) , this would be my recommendation
