You can't chose your doctor at  government hospital like Queen Sirikit. They'll send you to one of their orthos.

 

If you want to chose a doctor with expertise in shoulder injuries, and given that I gather you are in Pattaya and likely in no shape to travel (the best shoulder specialist in the country being in Bangkok) , this would be my recommendation

But it will cost considerably more than at Queen Sirikit

 

As to whether surgery is necessary it depends on where exactly the fracture is (scapula, clavicle, humerus etc) and its type (displaced/nondisplaced etc).

 

If you are being treated at Queen Sirikit and surgery has been advised, get the Xray films and go to the doctor listed above for a second opinion, consulation alone will not cost all that much (about 2000baht). Or if you are up to a trip to Bangkok this is the very best shoulder specialist:
