Departure from Thailand with required departure health declaration form

I understand a health declaration form is required before departing BKK to YVR transitioning through Hong Kong in July.

I downloaded the AOT app but cannot find this departure declaration form only the arrival form.

Can anyone provide a link for this departure health declaration form?

You need to check and comply with your transit and destination countries as you may be asked for and required to show documents upon check in.

Cathy Pacific website appears to state declaration form is required as per below...
