As posted elsewhere, from my experience, last Tuesday, at CW with an online appointment: check in at L27 before you go to L32. The officer there will check your documents and then escort you to the L32 booth. All very efficient. A few more forms to sign (one was to acknowledge penalties for overstaying).  The return of passports with extension stamp seems to be done differently from last year and took a lot longer but that might have been just my experience on that particular day. My bank just gave me photocopies of the last 12 months of transactions for the a/c in which I keep the 800,000 baht, along with the letter. Pleased with the online appointment system and very glad not to have to do that stressful early morning queue thing, followed by the rush to get the ticket.
