It cost me 50Baht for the lady at the reception desk to take a copy of my passport and whom ever to write the health cert. Best five minutes ever. There was just a little girl in the clinic .The doctor had been in and signed a pile of papers and left. She just added my name took my 40B and handed me the paper.
