 No online appointment in Pattaya, had to go there and make an appointment. 

 

All papers in two copies for two licenses. 

Copy passport, copy visa(extension) 

Copy old license 

Residence letter. (copy was good for second one.) 

Medical certificate 

Proof you've seen the online video
