80 coming very soon. For several years have to get up 2 to 4 times every evening to urinate.

Experiencing many of the usual symptoms; want to pee but nothing / very little comes out... I think it all done but within a couple of minutes need to urinate again, etc.

A few weeks ago had a PSA blood test. Result was below the usual number for my age.

I plan to soon get a full testing and hopefully some treatment / medication (if that's possible).

Any members successfully using any medications? Or can you recommend a suitable doctor in Chiang Mai. Please share.

Many -- I would even guess most -- AN members are on medication for benign prostatic hypertrophy (BPH) which is most llkely what you have (but need sowcialidt to xonfigm).

In terms of CM urologists I havd had good feedback on https://www.bangkokhospital-chiangmai.com/en/doctor/dr-wittawat-rawiyotai-m-d/

Agree. Speaks very good English, professional.Had him for three or four cystoscopies.

Finasteride/doxazosin or Dutasteride/tamsulosin both work for me. The dutasteride option is about double the price, though.

OP, from your description I would be pretty certain you have an obstructive prostate. I have one too, medication controls it to once or twice a night, with free flow.

There are 2 classes of medication used for BPH:

1. Alpha blockers like tamsulosin, doxazosin and other "...osins". These work by relaxing the neck of the bladder making it easier for urine to pass. Takes effect almost at once. These drugs tend to lower blood pressure so if you are onBP medx may need to adjust dosage.

2. Finasteride/dutasteride  and related drugs reduce prostate size over time. Takes 6 months or more to see results but will help prevent progressiln of yhe condition.

Usual to take both tipes of drug. There are preparations that combine them into 1 tdblet but this is more expensive than taking two separate pills.

3. Cialis (Tadalafil) 5mg daily. Same caveat about blood pressure as the alpha blockers.

The side effects of Finasteride (basically, chemical castration) and Cialis (an ED drug) are quite the opposite. If combined, Cialis helps against the ED (you want but cannot) from Finasteride, but not against the loss of libido (you are not in the mood) from Finasteride.

I had the same problem 4 years ago and had scans  which revealed no cancer thank goodness just enlarged prostratewhich was pressing on the urethra which is the tube that carried the urine out of the bladder. So I had a procedure called H.O.L.E.P. to shrink the prostrate..all good now.

I have been to see a urologist several times here in Pattaya.  I had been getting up to pee 4 or 5 times a night (!) for some time.  (Age 70)  At times, I also had difficulty in starting a urine stream while at the toilet.  The doc put me on the medication tamsulosin (0.4mg), after which I had no problem with initiating a strong urine stream.  However, it didn't help a lot with getting up to pee at night.

An ultrasound showed that I had a very enlarged prostate.  The doc then added the med dutasteride (0.5mg), which over several months decreased prostate size by 25% (also, !).  With reduced pressure on my bladder, I now get up to pee only once or twice a night = I'm very content with that.  So, I take both tamsulosin (410 Baht for 30tabs) and dutasteride (670 Baht for 30 caps) once a day (generics).

I tried the tablet that combined both into one tab, but found it better to take as two meds rather than one.  (The 2-in-1 med was actually 200 Baht cheaper than taking the 2 meds individually at 880 Baht for 30 tabs of a generic called Duodart.)  I have my meds delivered to Jomtien for free from Medisafe Pharmacy in Bangkok.

By the way, I also had the MRI for prostate cancer screening / result was negative.

Hope this info helps...

Thanks to all members, your comments and information shared very helpful.

...also, avoid drinking too much coffee. It's a diuretic.

Well I drink 2 medium size mugs of fresh coffee every morning. Nothing later, but I will go back to 1 mug every morning (7.00 - 8.00 am) see if that helps. I guess Lipton tea same affect.

Does that happen daytime too?

I've tried these meds for BPH but had severe reactions to them.

I now use this herb > Saw Palmetto. I've read that German Dr's subscribe it.

I still wake up to pee but way less often.

Lazada and Shopee have it. Or try (iherb) I've never seen it for sale retail in Pattaya.

I totally get how annoying that must be. I had a similar issue a while back and found that getting a thorough check-up was key. My PSA levels were fine too, so it wasn’t that, but my doctor put me on some meds that really helped. I’d suggest talking to a urologist about alpha-blockers or other options—they really made a difference for me. As for finding a good doctor in Chiang Mai, you might want to ask around in local expat groups or forums. They usually have some solid recommendations for specialists.

Any member aware if Rajajet Hospital in Chiang Mai has a specialist in this area?

Thanks.

They will surely have ond or more   urologists, all hospitals do, but I can't  recommend a specific one.

Really suggest you see the doc I recommended at Bangkok hospital.  As you have already gotten a PSA only likely test is a urinalysis which in inexpensive.

Whetever you go, don't buy medication at the hospital, these meds ard sold without prescription at pharmacies.

More likely your bladder had just become de-sensitized to fullness.

Note that may mean you still have urinary retention. Which puts you at risk of urinary infections.
