living in ko phangan, can i do 30 day visa extension in surat thani?

You can but you may have to wait to the next day for picking it up. Isn't there a Koh Phangan immigration office anymore where you can do the extension?

just go to the immigration offices on the island, they deal with them there 

but can be busy so get there early if possible

As long it is the same Province  it's not a problem.
