"Why are you here" almost certainly means "what do you need?", as in "why are you here in the pharmacy?". Not every Thai speaks the queen's English.

I never get infections, I only have sex with my girlfriend.

I had a motorbike accident earlier last year, stupid eastern European woman put me in hospital. I got stitched up, plaster cast and pain killers, I never got the prescription filled. I didn't need them, not required, not much pain.
