I need a medical checkup certificate of something like that to enroll in a summer university program.

What is this and where do I get one fast and cheap? I assume its just basic bloodpressure and stuff right?

Do I need to go to a hospital or can I get it done by a doctor? Any recommendations?

It is a very simple one page form, can get it at any hospital or clinic.

When I need it I just walk into a very small local private hospital near where I live, it takes about 20 minutes and costs I think under 100 Baht. You could also go to a government hospital, district hospital would involve much less waiting time and can do this just as well as anywhere else. I'd avoid provincial and tertiary govt hospitals as not worth the wait and also avoid the buig international hosps as not worth the added expense, unless you are in need of a real check up anyhow.

If you live in Bangkok, can you recommend a small hospital near On Nut?

Afraid not, dt live in Bkk but for this any old place will do (unless you actually need/want a "real"check up), I'd just walk into the first place that doesn't look too busy.

You will need to have your passport with you for ID

Any clinic can do it for you within 5-10 minutes, don't bother waiting in a hospital.
