Don't know about Chiang Mai, but should be possible to get it done at any hospital. Depending on what tests you want (or what you think you've caught) you might get the tests done for about 300 baht at a government hospital. You might be best off going in the morning with a full bladder (don't take a leak before you go there) so the urine tests are not diluted. Urine tests should be done mid-stream, just in case you don't get that information relayed to you clearly.

Most test results will come back quickly - just a few hours - but for cultures you may need to wait a day. If the doctor suggests you've got some soreness through rough sex (or vigorous "self-help") and don't need testing, insist you get tested anyway. Oh, one more thing: if the doctor or nurse asks you if you enjoy "tee-ow" (เทีี่ยว) they are talking about sleeping around and not travel.
