Re-entry Permit What To Submit?

Could I please verify the requirements to obtain a Multiple Re-Entry Permit?

Must go to local office as designated.

TM form.

1 Photograph.

Passport

Photocopy of Passport information page

Photocopy of most recent arrival doc

Photocopy of Passport page pertaining to type of permission to stay. (Retirement in this case)

Looks good to me. They will tell you if anything else needs to be copied. They usually do.
