When I paid some years ago, it was the office right on the river. Maybe it is called the Mae Ping branch.

I thought the instructions included where to pay.

From Q & A on US Consulate Chiang Mai site:

I used the Sam Yaek Suan Prung Post Office just down the road from Airport Plaza and Niyom Panich heading towards the moat. Purchase of a return envelope for passports though has to be done from the Mae Ping post office next to the Nawarat bridge which has been mentioned by others.
