My 60 days are up on the 22nd ( monday ). Just wondering what I need to get before then? And what's the best time to arrive?

Ask them at Immigration which form to fill in, pay 1,900 Baht, passport with signed copy and wait. I think that's it....but I'm sure someone has a very recent experience and will be along shortly.

For sure you 1 copy of your main passport page. 1 copy of your visa page. 1 copy of your departure card. Plus one passport size recent picture of yourself. Then of course the important 1900 bahts. The form to apply for the extension you can find just inside the immigration office. I believe it is form #7
