In order to get the non-o he would need documentation from his dentist or clinic in order to apply for the visa. He most likely would only get a single entry which would not allow enough time with out getting an extension at immigration for medical treatment (questionable).

Update from OP. Received my passport with Non-Imm O visa (single entry) from Portland today. Courteous service with less than a one week turn around. Highly recommend this office.

I already have a reservation for a 2 Feb flight to LOS. Will apply for a one year extension based on retirement after 60 days in-country.

Really appreciate the help from posters on this site. One last question. I am planning on a short trip to the PI in late Feb. Since I only have a single entry visa, how do I apply for a re-entry permit? Is that done at any immigration office, or only at BKK?

A Re Entry Permit can be got at the Immigration Office closest to you. 1,000 Baht and a photo needed.

They can also be got at the airport after check in but I would never fancy leaving it that late personally.

You apply at the immigration office for the area where you reside. Cost is 1,000 baht and you need to bring a passport photo.

Good information on this thread. By way of update, I renewed my US passport and was nervous about mailing it to a Thai consulate especially since I live right outside Washington. So I filled out the O Nonimm application for multiple entry, made a copy of a bank statement and added a copy of our marriage certificate and wife's green card for good measure. Then I went to the embassy to drop it off with my wife at my side. At first a young consular official asked to see our air tickets. I explained that our travel plans weren't certain and we hadn't purchased tickets and wanted the multiple entry. A supervisor came over and looked at the application and at us and said ok. Long story short - I had my visa 3 days later. Getting to the embassy is a bit of a pain, but ok if you go early in the day. This sure was easier than getting my wife's visa from US immigration!!!
