Ordaining at Monastery in Thailand

Hello everyone i don't know if i am posting this in the right forum but i just needed some help. I am looking to ordain as a monk next year i have looked at website which is all in english but the form i need to fill out is in thai and i'm not sure what it say could anyone help me please many thanks
