There is no form. The bank writes a letter confirming your balance. Most have a template for it and just fill in the blanks and print it it out.

The TT3 was replaced by the Foreign Exchange Transaction Certificate (FETC), but what is it that you need it for? If it is proof of funds when qualifying for an extension, then you need the bank letter Ubonjoe mentioned. I don't know if you need the FETC when applying for an extension based on investment.

If you are buying a condo or want to be able to re-export the funds, then you will need the FETC (but in that case you posted in the wrong forum) .
