Did they give me the right form?

Applying for a extension of tourist visa and as a condo owner she gave me a form titled

Notification form for house master owner or the possesser of the residence where alien has stayed

The second page asks for the name of the alien, so do I list myself as the alien?

mcfish, you submit the TM.30, because you are the House Master, ie the chief possessor of the residence in your capacity as the owner and resident, as defined in section 4 of the Immigration Act.
