register Logo/trademark Where do I get the application form?

I need to register our new product trade mark, copy write and logo as well as a older one we have used for years.

I DO NOT want a lawyer or law firm to do it, last time I had them do it. The trade name was leaked and some one beat me to the registration than tried to sell it back to me.

I am the only one who knows the Trade marks and logos as well as new trade names.

They will be registered to my company.

Even though the final approval takes time, the protection begins from the date and time of the application.

Would appreciate it if some one could tell me where I can download the application and where it can be submitted either in Koh Samui or Suratthani.
