I've tried reading the 'Pinned' post for an answer but can not find one..... So apologies for posting as new topic.

I have a I obtained a Retirement Visa from Jontiem Immigration office a month ago, but didn't have Re-Entry Permit.

What forms & what documents do I need to apply for either Single or Multiple Entry permit - Also what are the costs ?

You apparently have an extension of stay based upon retirement not a visa.

You will submit a TM8 form with a 4 X 6 cm photo attached, Download: http://www.immigration.go.th/nov2004/download/pdf/tm8.pdf

You will need a copy of your passport photo page, visa, entry/permit to stay stamp, extension of stay stamp and TM6 departure card.

The fee for a single re-entry permit is 1000 baht and a multiple is 3800 baht.
