If you are dealing with CW, I suggest you obtain these items day or two prior to day of application.

They MAY approve a joint account with double the financial requirements for extensions based on retirement (as per the OP)

Just for general information: there is basically nothing in it for agent.
The charge ~ 4k depending on age of client, goes on "accident insurance", ATM card and minimum deposit into the account (say 500baht)

Since you are based in Bangkok, you didn't consider land crossing border bounce agent from Bangkok or Pattaya to Cambodia
For DS-82 passport renewal it is done via mail.
