As you age/deteriorate you get as much support as necessary, to the worst case with Alzheimer's where you can get 24x7 one on one care if needed. No need to try to entertain the wife/gf. No need to worry that the wife/gf will panic when you have a crisis, etc. etc.

Your body changes as you age. Heart gets weaker, arteries blocked, etc etc. The symptoms you were treating 30 years ago may no longer exist so your meds need to be adjusted. I took 2 or 3 BP meds 30 years ago and now I need none. Just an observation, but your a1c of 5.x is low for a diabetic. This is not necessarily an issue but the normal target is 6.x so you might be able to reduce some of your diabetic meds too. More body changes as you age.
