Friend here  on a 30 day visa on arrival. If he wants to extend for 7 days, does it extend his  existing visa or does it start counting 7 days from the day you extend it?

Also, if the 7 days are added on top, how many days can you get it before your existing visa expires?

Thanks

The 7 days start from the end of the permitted to date not the day you apply

With most Immigration Offices it's 7 days before that you can apply, some may be up to 14 days

Im pretty sure there is no 7 day extensions just 30 days

The 7 days is given for a "refused extension of stay" - they give you 7 days to leave the country.  It costs 1900 Baht, same as any other extension of stay.

It is most frequently by those on B (work) based extensions whose job ends, to give them more than a day to gather up their things and leave the country - though can be utilized in any situation where one's permission-of-stay is ending, and no extension is possible.

There is also a 60-day extension to visit Thai family (wife, kids).

I am lead to belive this has been done away with  sure UJ will expand on it soon.

The 60-day extension to visit family is still available under the currently valid Police Order.

if the op's 'friend' is on a 30 say visa exempt entry then a 30 day extension will be issued, which will start the day following the last day of his 30 day stamp, thereby giving him 60 days here.
