Visa forms help required.

Going to Savannakhet, downloaded the 2 forms, visa on arrival and visa application form. Decided to fill hem out before going to make things easier and glad i did.... they are a bit confusing. I will be staying in Mukdahan. Getting a non-O multi entry visa.

 

Visa on arrival form :- 1. Purpose of visit ? 2. place of residence in respective country (just say's - Tel) 3. name and address to contact in Laos ? (who ?) 4. Proposed address in Laos ? (leave blank ?)

Visa application form :- 1. section 2, Present address (Laos (leave blank ?) Permanent address (if different from above) (home address ?) 2. All of section 5 3. Do i have to sign section 6 as i'm getting a non-O ?

 

Where do i put that i want a multi entry non-o visa ?

VOA form.

1. tourism or apply for a visa

2. Telephone number and beside it is country which could be Thailand.

3/4 Hotel you will be staying in.

Visa application form.

1. Lao address can be your hotel.  Permanent address can be your address here.

5. Lao leave bland and Thai yourself or your wife.

6.That is a notice. You are signing the application form not acknowledging the notice.
