Work Permit Medical Form REFUSED

Hi,

Just had a huge surprise, have been doing my work permit now for over 20 years and today I submitted my new medical form which was always the same since years and the staff at the office have refused it because it does not have a box to "tick" that I am not an alcoholic. 

 

I checked my old forms and none did, does anyone have the supposedly new form which requires a tick next to not being an alcoholic?

Would appreciate any advice or if you've a form I can download it would great.

Clinics tend to print out their own work permit health forms. You could always ask your clinic to change their form.

 

Whereabouts are you located?

Was it your own office staff or the labour dept. that rejected it?

Please find attached 'Health Certificate for Work Permit' used by Bangkok & Chonburi offices as of December 2019.

 

Our local offices now stipulate that the heath checks must be carried out only at a Government Hospital, not a street corner 'quack/clap' clinic ????

I just had a WP health check and the only thing the doctor did was ask whether I was an alcoholic (and syphilis test).
