Another 60 day extension after Nov.25th.

I have a valid multiple non immigrant (O) visa, valid till March 2021.

Went to CW immigration on September and received a sixty day extension until November 25th. due to seeing my daughter.

I have seen a post saying it is now possible to get another 60 day extension.

Is this correct?

If so my questions are:-

1.The last time, my daughter had to accompany me and paperwork included birth certificate, ID copy, rental agreement etc.,

    Is this required again?

2.I see an affidavit needs to be filled in.

   Are the questions just a formality, or is it scrutinised, requiring extra back up paperwork?

3. Is it possible to download this affidavit form.

4. What other paperwork is required?

5. Is the cost 1900 baht?

6. Has anyone actually gone to CW immigration and have first hand experience of this procedure?

7. How many days in advance of my visa expiry date of November 25th. can I/ do I need to go to CW?

Answers to any of these questions would be really appreciated.

Also any other information that is pertinent.
