What type of visa entry did you have before applying for the extension based upon medical care?

If you entered using a tourist visa or got visa exempt entry you could apply for a 90 day non-b visa entry at immigration.

Arrived on a M/E Tourist Visa.... then Amnesty etc.

You can do a change of visa status (TM86 form) at immigration to get a 90 day non immigrant visa entry based upon working or teaching and etc with supporting documents your employer. Then near the end of the 90 days you could apply for a one year extension of stay.
