US Thai Embassy COE (Cert of Entry) website useless, anyone else?

I've filled this form out 5 times. I've tried Chrome, Firefox, different image formats, everything. When I click submit all I get is this modal saying I need to attach a copy of my passport:

I've I've followed the entire form to the T, including attaching my passport. Is this happening to anyone else?

Are you clicking on the Upload button after the passport image is displayed on the screen?

As @treetops said, you have to click on the little upload button after you have selected the file.
