(also please let me know if you think that the template will be refused by the Thai authorities)

Shouldn't be a problem being accepted (personally I'd highlight the "Fit to fly as a normal seated passenger" line), most reports I've seen don't use that form. 1

In the UK it is not a required document in order to get a visa or a COE. The fit to fly is presented to airport staff at check in.

Robin Posted February 12, 2021 Posted February 12, 2021 Arrived in Thailand 3 weeks go from UK. Fit to Fly Cert was issued by UK clinic that did Covid test. Simply letter from Clinic headed fit to fly, with Covid test result attached. Nothing like the F2F cert shown by Thai Authorities. printed out letter and Test result cert and showed both at Check-in and transit airport. Accepted without question. I think the original F2F cent was something TAT picked up on the internet, and applies for any sick passengers, but nothing to do with Covid Covid test done in UK, at My Health Care Clinic near Heathrow. found on Internet. They advertise Covid test and F2F cert as package

I note that they ask for Quote The important information that needs to be shown in the Fit to fly certificate are – Letterhead/ Logo showing the name of the clinic/ urgent care – Name of the Doctor/Physician/Nurse/Healthcare provider – Medical license number + I'm guessing a mention that covid test is negative

Me and you both, I wouldn't be chancing blowing the time spent getting all my ducks in a row on that document (person who asked about it being OK was advised getting another more "professional looking" certificate which I assume they did). 1
