Lightangel000 Posted May 8, 2021 Posted May 8, 2021 Hi thanks for kind help I expect something went wrong when my father died at the hospital. I visited the private hospital before covid, got dads medical journal - but his death certicate was never included with time of death. The hospital write to me not wanting to give out the directors name for the hosptial when I enquire about his. I am from Europe where documents are written by computer and doctors must stamp with their registration code for being a doctor with the medicinal registry so no fake doctor can write certicates. I got my fathers death certicate from a private hospital in Thailand: it is written by hand, I can barely read it - its in a form with squares, filled in, and the doctors who wrote it has no stamp, no registered doctors number by his name, and the paper is not an official form from with the hospital logotype on the hand written death certicate with time of death. Is this normal? Something I expect went wrong at the hospital, as they did not include his death certicate written ¨by doctor in the medical journal I payed for years ago. TIA
Sheryl Posted May 8, 2021 Posted May 8, 2021 Moved to the Health Forum as not hiemn country related. In Thailand doctors do not usually record their registration numbers on documents. But the certificate should be on an official form. Are you saying the entire thing is hand written? Can you post a pix of this document with the identifying info blocked out? It will be easier for us to advise if it looks genuine that way. Please also clarify how you received this from the hospital. Was it handed to you, personally, by the hospital? Or is there some other person in the middle here/
