For US travellers - Do you just upload the CDC vaccination card for Thai Pass

For US travellers. Want to visit Thailand in around May-July after I get my Tourist visa. To obtain a Thai pass and to provide proof of vaccination, do you just upload the CDC vaccination card? Read somewhere that the vaccination proof must contain some kind of ID matching name and birthday. The CDC card does not have any ID only name and birthday. Can somebody from the US clearifies who has obtained a TP from US by uploading CDC vaccination card. Thanks.

You CDC vaccination card will be accepted to apply for a Thailand Pass. Many people have used them to apply for a Thailand Pass.

Also check if your provider (Walmart, CVS etc.) or state health dept offers a smart card or QR code.

 

Yes, a photo of the CDC card was sufficient for getting my 3 family members their Thailand Pass. Maryland, at that time, didn't have the capability of providing a SMART Health Card QR code. I believe they now do via https://app.myirmobile.com which is used by several states.

Thanks everybody. Yes I verified my state (IL) has QR code and that I can scan into Smart Health Card verifier app. DO I print out a copy and then upload it into the TP website? I assume so.

I've obtained 2 Thailand Passes using the CDC vaccination card. You can also get the QR code if you go to https://myvaccinerecord.cdph.ca.gov/

And complete what you need to

I just got a Thailand pass approved yesterday using only a photo of my US CDC card as evidence of vaccination.  None of the fancy QR codes are necessary.
