Pain on left testicular and severe back pain

Hello Everyone,
I noticed a pea size Lump/swelling on my left testicular about three-four months ago. Suddenly I saw the lump and went to the doctor at a local hospital. The doctor gave me antibiotics and I took two times antibiotics. The lump was gone. I did three times ultrasound and the doctor suspected that three was an epididymis pea size cyst on the epididymis. Previously, I posted my problems here to get the suggestion of a good urologist. After getting suggestions from the local experts, I went to the Bumrungrad Hospital in Bangkok to consult with the doctor. The doctor just checked and said there is nothing wrong. He didn’t give me any diagnostic test to check further. He said everything is okay and the swelling might be a fluid of spam. He also said back pain might be for other reasons. Previously, I did an ultrasound for the kidney, and bladder also. Everything was okay.
But now I feel dull ache pain on my left testicular and severe back pain. Sometimes I feel shortness of breath too. I also feel metallic test and dry mouth. I am really scared and concerned about it. Even now I feel pea-size swelling outside of my testicular. But I am not sure whether it is a general structure or anything wrong. I am concerned about it. Could anyone here experience this type of problem? Please share your experiences and give me suggestions. Please suggest to me a urologist if you think I should go again. Yes dull ache at times, but I have been vigilent in preventing any accidental external pressure when sitting in uncomfortable positions or in restricted clothing.

I have had many back pain issues but non asscociated with testes

I had clamydia in my younger days with same symptoms (included back pain) not necessary the case here, but good to know.

Yes, tested positive for chlamydia, and before test result was clear given antibiotics and after that never had a problem again. 
 

Todays std’s can be resistant to antibiotics, so better check again if not tested.

As my father died from testicular cancer I also was being monitored by major cancer hospital in Australia I had my oncologist examine my lump.

Please don't take my experience as a yardstick for your issue, definitely go to hospital for definitive diagnosis

Australian oncologist diagnosed it as fluid sac nearly 50 years ago, it's still there no problems

Same for me, it was almost as if I had three balls. 40 years on, no problem but it's still there.

The various symptoms you mention do not have a common cause, unless it is stress. In particular, shortness of breath, metallic test and dry mouth and nto related to your testicular issue nor your back pain.

 

Epididymal cysts are harmless and require no treatment unless causing pain. Rarely, they can cause back pain. So can epidimitis.  But back pain can also have many, many different causes.

 

If there is now a palpable mass on your testicle, go back to urologist. It sounds like the mass was gone when you last saw him.  But try to relax, an epidimal cyst is not in any way serious.

@SherylThank you so much for your response. Last time, following your suggestion I went to the bumrungurd hospital and meet with Dr. Viroj . Doctor checked my testicular properly and said nothing is there . He noticed swelling. But he said it’s normal and might be fluid of spam . He also said the back pain might for another other reasons. 
 

However, I feel now dull ache pain on my left testicular and severe back pain . Last two days , I also felt little bit shortness breathing. But

now it’s okey . I feel pea size fluid outside of testicular but I am not sure it’s any organ of structure or any cyst. And back pain like comes and go . I just scared due to back pain and also testicular pain . Bc I read many things from google. That might make me more scary. 
 

now I am planning to meet the doctor again on Saturday. Hope everything will be okey . Thank you ???????? so much for your time to response and suggestions.

The faster  you see a doctor the faster you get good  pain relief. You can choose your  doctor  BTW. I changed my gastroenterologist at BH, nobody  seemed surprised at my request- I was glad I did. Speak calmly and clearly to the  doctor. Express  your worries too him. Make  your concerns clear so he can address them. Write stuff down  if it helps you remember. Do uou have a Thai  girlfriend  to go  with.

Its a dread feeling a pain in the bolleaux because  it hurts, can hurt massive! It's embarrassing and it's worrying. It will turn out to  be nothing important  but meanwhile STOP GOOGLING SYMPTONS and practice  deep  calm breathing to reduce panic  and anxiety- just simple deep slow abdominal  breathing. Tell the doc  about your  panic/ worry and  consider  a 5 day 2 mg course of Valium  to break the panic  cycle and stabilise  breathing.

Dr. Viroj at Bumrungrad is very well respected.  Twice the OP wrote that he said the swelling was "fluid of spam".  I googled that and got no sensible results.  Perhaps there is a misunderstanding of what Dr. Viroj said.

I think a typo (or misspelling) and he meant soerm not spam -- spermatic fluid.

@Sheryli have gone to the doctor again. But this time I went to the Bangkok hospital. I consulted with One of the urologist doctor in Bangkok hospital. I told the doctor about my severe back pain and dull Ache pain on left testicular. He checked my testicular and said there is nothing. I requested him to give any diagnosis test (ultrasound or any other test)  to identify the problem. But he didn’t give any test and just prescribed pain killer medicine. 
I told him about breath problem and metallic taste on my mouth. But He said that he doesn’t know About metallic test . The doctor also said it could be chorionic pain and

reasons might unknown.  
I felt pain still and went to the SWU yesterday again . I will do another ultrasound on 25th of this month  . Doctor prescribed me antibiotics for 7 days . Before I took two time (14 days antibiotics) already. So I Am concern about taking antibiotics again . Is there any problems if I will take antibiotics again for 3rd time ?

I still feel back pain, mild pain on

left testicular and enlargement of my breast . Sometimes I felt breathing problem and heart compounding . I don’t know whom should I go and can identify the problem and diagnose properly. 
kindly advise me if you have anything for me .

At some stage you are going to accept a diagnosis that suits your beliefs, why not accept that several doctors have checked your teste and found your concerns to be unwarranted . Relax, take the antibiotics they may get rid of any infection the doctor diagnosed. Buy a general health checkup set at any hospital to ease your concerns

There us nothing to suggest a need for antibiotics.

 

What is SWU? 

 

I think you need to accept that multiple urologists  (and several scans)  found nothing wrong with your testicle.  Also need to accept that there is no single underlying physical  disease  that accounts for all your symptoms. And that at least some of them may be  anxiety related. 

 

Back pain can have many causes and best to see an ortho specialist if it persists.

 

Sensation of trouble breathing and heart pounding may be simple anxiety but to be sure have a general check up that includes EKG, chest Xray and thyroid panel. 

 

Regarding metallic taste that is usually due to medications of some sort. Please indicate all medications (including supplements) that you take.

 

Also have you taken a  COVID test recently?

SWU meaning Srinakharinwirot University hospital at ongkharak. 
 

yes, I got Covid last month (18 July) and already recovered after 7 days . 
I took only medicine that was given by doctor for Covid . I didn’t take any other supplements and medications. 
 

Now doctor (Urologist from SWU) prescribed me again antibiotics for another 7 days. But I haven’t taken it . Doctor (Bkk hospital)  prescribed me pain killer medicine but I didn’t take it . 
 

since yesterday, the pain on left testicular is mild but back pain still continuously experiencing. 
I have attached  the picture of medicine for your reference. I didn’t take these medicine yet. Please suggest if I should take antibiotics.

COVID after effects can  last for weeks to months. Even in people who's case was very mild.  had COVID around the same time you did and am still feeling after effects like tiring easily and becoming short of breath on exertion. 

 

Metallic taste, occasional shortness of breath and palpitations can all be after effects of COVID.  If these symptoms become constsnt or severe then see an infectious disease specialist (not a urologist) or internal medicine doctor. Otherwise just try to get plenty of redt, exercise regularly but in moderation (listen to your body as to when to stop) and try not to focus so much on it.

 

Back pain is less likely to be due to COVID and can be from muscle strain/spasm, excessive sitting etc. Sometimes due to issues in the spine especially in older people but since your back pain does not sound that severe usual approach is what the doctor did: treat for inflammation with antinflammatory (Arcoxia). As long as you have no history of heart  problems or liver/kidney impairment or gastric ulcers and are  not over age 65, a short course of this as prescribed should be OK and may help. And avoid sitting for prolonged periods. Get up and walk/stretch at hourly intervals. 

A heating pad may also help.

 

If the back aches become severe or if the pain starts to radiate into your thigh or leg then consult ortho specialist. 

 

Regarding the antibiotic as I understand it you have already had several course of antibiotics and no infection has ever been identified. And specialists previously consulted did not recommend further antibiotics. I would follow their advice i.e. not take the Cipro at this time.

Now I got more clear suggestions what should I do next for the diagnosis and identify the back pain problems and others .

With this comment and suggestions I make no reference to the testicular element of the condition, or known abnormal spinal problems

 

For anyone with back problems that are most common in the lumbar region (the lower back) it is a useful thing to remember that proper supportive shoes can make quite a positive impression for the relief of back pain. in addition to which helps improving posture.

 

Your regular seating needs some kind of (lumbar) support that "pushes" that lumbar region towards the natural shape of the spine. Try a fairly hard cushion on your chair and use it in your car also for driving, Initially, you don't need to buy anything special.

 

Your bed should also be appropriately supportive for your back.

 

One more thing, when bending down to pick up anything (especially if it is heavy) bend your knees and unbend - letting your knees do the work - on rising.

 

These suggestions pre-suppose your back isn't in need of clinical treatment.

 

I am 77 years old and have practiced these things for more than 40 years. I don't have any back pain!
