Extension of retirement visa

Do I need to show proof of health insurance now to extend retirement visa

Which Visa did you enter the country on was it a non imm "O" or a non imm "O-A"?

If you entered (or converted in-country) to a Non-O Immigrant Visa, the answer is "no"; however, if you entered with the Non-OA, then "yes."  You can check other threads for the insurance requirements.

Then yes, you need insurance.

As others have said yes with O-A you need insurance

But I wonder if it might be better to use an agent & switch to O retirement which does not (if that switch is possible)
