I noticed that the form for the Re-Entry Permit asks for the departure and return dates. I'm wondering if these dates are enforced or can be changed later.

 

I'm planning to leave on a one-way flight and the return date is not yet clear. I can make a best guess to fill out the form but what happens if I return on a different date?

That's what I'm thinking. Is the Re-Entry permit a stamp or a piece of paper they staple to the passport?

It's a stamp. It's valid from the date of issue until the expiry of your permission to stay.
