Certificate of residence: What is needed?

I have just returned from the amphur to check my documents for a yellow book application. They asked me to get a certificate of residence from the Immigration office. As i have always had a work permit and never needed this before, what do i need to get this? I have a non O but i do not have a 90 day report.

You cannot obtain certificate of residence at CW without 90 day report.
