I'm applying for a UK Returning Resident visa, and I need to supply VFS Global in Phuket with a TB test.

only two approved clinics in Bangkok...

 

So I can't just walk into my local branch of Bumrungrad Hospital and get a nicely certified TB test on official letterhead from a doctor after an X-ray; it's not allowed, right? Have gotta go by a boat->fly->taxi to a clinic in Bangkok? What......

I don't think anything is implied. It is a requirement.  The reason that the test can only be done at the approved clinics is because it would be too easy to obtain fake certificates elsewhere.

My mate got one from Dr. Lek on Sukhumvit Soi 14; he says it's legit as its gotta a stamp, must be.

It's not legit. May not even be genuine.  If the idea of the TB test is to get a UK visa, then good luck. The visa will be refused if it is not from an approved TB test centre.
