Customs ID Card

Does anyone have information on how to apply for the import/export company ID card for customs. I think they used to call it the customs sample card. We used to have one for import in my wife's name but it expired 24 years ago. I would like to get one now, preferably in my name (farang), to make it easier to claim export exempt VAT rates. I'm in Bangkok and suspect it needs a trip to the Klongtoey office, but I would like to have the form and supporting documents ready before I go.
