For several years I have been taking 1 80mg tablet of Diamicron at 7,30 ever morning. A few months ago the local pharmacy announced that they no longer stocked Diamicron but gave me 80mg tablets of Clazide instead, assuring that it is identical to Diamicron, only made in Thailand under licence. Certainly, the small print states that each tablet contains 80mg. Glicazide.

  However, since taking it, I have noticed a more frequent occurence of more severe hunger pangs accompanied by sweating at about midday. This is rapidly cured by eating something. Is this due to the change in medication? Would I be better hunting down a source of Diamicron?

Was the Diamicron you were taking immediate release or modified release? Makes a big difference.

 

I have only very limited information on Clazide but it looks like it is immediate release.  Should never switch from modified or   extended release medication to an immediate release form without first consulting a doctor as dose and frequency may need adjusment.

 

There are 3 forms if modified release Glucazide available in Thailand but all are 60 mg not 80 mg

 

Diamicron MR 60 (60mg not 80)  imported

Glucazide MR-60 ( 60 mg,  locally made)

Glycon MR (also 60 mg locally made)

 

There are also combined metformin and glucazide preparations. 

 

I am puzzled that you say you were taking Diamicron 80mg. While it exists,  it is not registered for use in Thailand, only the 60 mg MR formulation. Do you happen to have a picture of the box? 

 

If you were in fact taking 80 mg of glucazide daily but are now experiencing  symptoms of hypoglycemia at same dose, and if both were immediate release preparations, then your requirement may have changed which is not unusual with diabetes management.

 

Risk of hypoglycemic doelld is alwsys greater with immediate release preparations. 

 

The best thing to do is to consult a local  endocrinologist or internist for advice. They will be best positioned to   know what medication preparations are locally  available, how their actions compare, and to advise you. Bring recent lab results if you have them. Where in Thailand do you live?

An update. Sheryl s suspicion that Clazide is an instant release medication is reinforced by the fact tht the tablet starts to disolve in my mouth before I get the chance to swallow it.

I went back to the Pharmacy and spoke to the duty Pharmacist. She was unable to conform that Clazide is instant release, but produced a leaflet which instructed that it was to be "taken with food ", I had been taking it before breakfast, (i.e. on an empty stomach), as I had been doing with Diamicron. I changed to taking Clazide at the end of breakfast and have experience only one low sugar episode in the month since ( after a strenuous 15km cycle ride.)
