Theres a new format for the thai visa form. I'm applying for the multiple entry non-immigrant o visa and the form has a space for number of entries requested. Do I need to pick a number or can I just fill in multiple? Also what is the local guarantor as opposed to the one in Thailand? The example form on Thai Visa is the old one and needs to be updated.

Okay, thanks. Also should the reference person in Thailand on the additional long stay form be the same as the Thai guarantor on the main form? In addition theres space that says spouse (if appalicable) on the long stay form but it doesn't indicate what information about the spouse is to be included. Should I just write yes?

Where are you getting the term "long stay"? That is used for non immigrant OA retirement visas only AFAIK. You said this was for a non immigrant O visa. They are not the same thing.

Ok, on closer inspection I realized that the long stay form is just for retirement; I had thought it was a supplement to the visa form for those needing any kind of non-immigrant visa. It's good that you pointed that out or I would have sent it with the other paperwork causing confusion.

One final question: for duration of stay do I write one year beause it's a one year multiple entry visa and thats how long I plan to stay or just 90 days, the maximum stay permited on any entry within the one year time frame?
