You are perhaps thinking of Siam City Park, where a person was killed on the log flume a couple of years ago when it malfunctioned.

Why all of the comments ? Most of you should be aware by now that the Thai motto For maintainance is " Do NOT fix it 'til it's broke ",this seems to apply to practically anything and everything , probable reason being that no one gets taken to task (or rarely) for any consequences of this negligent attitude .
