It is given to you before arrival on the plane.

Also available at the airport should you be sleeping & miss the handout.

Dont know if it is down-loadable anywhere

As said, you can only use the card you are given on the plane. If you want advance information about what the card looks like, here is a scanned copy:

Don't bother, it just takes a minute if you wanted to pre work the form. BTW, the plane have many so years ago I grabbe a few for future use. I have lived in LOS for a few years now so I have plenty. FFWIW it is a short form and you can do it on the plane>>>>>>>>>>>>>>

Inbound passengers get them on board the airplane and can also get them near immigration.

Outbound Thai passengers get them at airline check-in and at immigration.
