You will need copy of passport date page, visa/entry stamp and arrival card and 4x6cm photo taken within six months plus 1,900 baht to pay for TM.7 application submission (and passport for stamp to be entered into). The address is end of Soi 7, Chiang Watanna Road. If you give directions it is building on right so go to end of Soi and turn right to center of the South side and enter on 1st floor drive (turn right and cross road and proceed up to entry). Immigration will be inside to the right (just before the open space).

When you enter the building, the visa office is on the right. You fill out a form that they will give you at the counter. Go downstairs and get your photo taken, I beleive it was 100 bhat and a few more baht for photo copy of passport visa. Go back upstairs and attach the photo with a glue tube that they have at the desk.

Then go to the next room, to the window and get a number, proceed to the waiting area and wait for your number to be called. It will cost you 1900 baht as the previous poster stated

FYI: they close for lunch for about one hour, so try to get their earlier in the morning or later in the afteroon

Not to late in the afternoon or you might just have to go back the next day.
