July 18, 201312 yr Hi, I need a proof of address to change the name registration of a bike. I know this is done at immigration. Now for 90 days reporting there is a paper you can download and fill out. I wonder if there is a similar paper to prepare for this so i have the least amount of hassle and make it as easy for bother parties (immigration and me) I ask this because my handwriting is not that great and I just want to fill it in nice and neat. Besides that I just like to come prepared. I have gotten this paper before but it is a while back and I cant really remember what is needed for it I would appreciate it if someone could help me out here a bit. Thanks in advance.

July 18, 201312 yr For driver licences, a Work Permit can be used for proof of address. Seems reasonable that it could be used for transferring a name as well.

July 18, 201312 yr Author Lopburi could you tell me a few things And my entry visa do they just want the date of the last extention or when I got the first what do they mean by conveance (do they want to know how I got here ?)(plane) And the last part visited at the royal thai consulate or embassy what do they mean with that.

July 18, 201312 yr Immigration has accepted the form I posted when I gave it to them. You will need two photos when you apply.

July 18, 201312 yr ^^^^^^^^Rob, you dont say where you will do this, you need to do it where you do your 90 day reporting. I did this recently at CW, there is no need for any photos, the cost is 200 baht, it can take up to 3 weeks for the form to reach you, it comes by EMS, mine took about 4 days. I used the first form Lopburi linked to. You will be asked for a photocopy of your passport and arrival card.
July 18, 201312 yr Author I will do this in Nothaburi immigration its near my home, i could hop on the bike and drive a few times as it is only 5km away. Last time it did not cost me anything and then I could pick it up in a few days. Reason I am asking for a form is that I like to have it in nice type instead of my handwriting. Just want to have it look good for them so there is the least of hassle. I have my passport and arrival card scanned in so its easy to print out. I am quite used to the Thai need of fotocopies by now.
July 19, 201312 yr You might want to check to see if they will do a residence certificate now. I think I recall a report that they do not do them.

The only form required at CW was the one in the first link, no need for the second link, the driving place on Suk Rd soi 62ish were happy with the form in the first link.
July 29, 201312 yr Author I can confirm they don't want to hand them out. They said only for a motorbike or car license. Not as proof of address to buy one. Offered money pleaded was persistent had the right documents nothing helped. The strange thing is before for my motorbike there was no problem getting this document.
July 29, 201312 yr Author You will need to get one from your embassy and have it translated to Thai. Not sure it needs to be translated as it will be in English of course. And the proof of address from the police wont be accepted, as we called about that.
July 29, 201312 yr It might save you a lot of problems to pay a few hundred baht to have it translated. The dealer and for sure the transport ministry might have a problem with a document in English. The one from immigration is in Thai.

Told them I would not mind paying for their service ect. But if officials don't want to do something as a mere mortal you can't change them even if you are right.
