Does anyone have a link to the form and maybe know what pages I need to copy in my passport. I need to get a re entry permit or multi entry permit. I want to make the application as fast and good as possible so I am asking for a form that i can already fill in and get the required documents.

Immigration is not far from my home but its better to do it fast and good.

When you fill out the form their is spaces for your travel dates. Put whatever dates there you want because they do not matter or just lleave them blank because office will accept the application with them blank.

1 question they ask about my visa when it was Issued do you i need to fill in the original date of the Visa as all others are extensions on the original ?

You put info for the visa that gave you the permit to stay that you have been extending.

I found this thread and got my first question answered, then another occured: I'll be arriving on a current ext of stay next month. My present address of record in Cha Am but I'll be going direct to Khon Kaen direct where I already have a new/long term address. I'll give that address upon entry. If I go to KK IMO for a re-entry permit, will I need to do a change of address. I will only be in KK about a month then another trip out of country and back to KK. I'm thinking the Imm records system may be good enough to already have updated the address via entry application at BKK. Thanks in advance.

You will need to formally change your address at KK immigration. Putting the address on the arrival card is not enough,

You should complete a TM28 form. http://www.immigration.go.th/nov2004/download/pdf/tm28.pdf

They will want proof of residence and possibley a TM30 form from your landlord. http://www.immigration.go.th/nov2004/download/pdf/tm30.pdf
