Tourist Visa and Extension to Stay - Phuket

A friend has a 6 month Tourist Visa with multiple entries.

As he understands it, he needs to report to Phuket Immigration after 60 days and apply for an extension of stay.

He arrived in Thailand on 6 August, 2014.

His questions are:

Is it correct that he needs to report to immigration at 60 days?

He has heard that he needs to report at least one week before the 60 days are up. Is this correct?

Which form should he use?

Which documents are needed? and

Is there a cost and how much is it?

He can apply for the extension a few weeks early, but on day 60 at the latest.

He should use the standerd tm 7 form

What he needs is a copy of his passport (dat page and visa stamps), a copy of his arrival card and a pass photo.

The fee is 1,900 baht.

is the report due 60 days from date of arrival?

It is not a report. It is applying for a 30 day extension of stay that can be applied for a couple of weeks before the 60 days ends.

If he did not do the extension he would have to leave the country on or before the 60 days ends to use the next entry allowed by his visa. How many entries does the visa allow their is no multiple entry tourist visa.

Sorry, I was misinformed. Not multiple entry.

it is 2 entries; one of which was already used when he first arrived on 6 August

After getting the 30 day extension he needs to do the 2nd entry before the enter before date on his visa. He should not be close to it unless the visa only had a 3 months validity from the date issue.

1] At the top where it says "immigration office" is that space for immigration to use or am i to write in there the name of the office i am visiting? If the latter, what is the name for the office at the Bangkok address of the following:

Government Center Building B, No. 120 Moo 3, Chaeng Wattana Road Soi 7,
Thung Song Hong, Laksi

2] In the space for "Reason{s} for extension" what do people usually write there, or what is recommended or acceptable? Simply one word like holiday or work?

3] On page 2 where it says "Tambon/Khwaeng" and "Amphoe/Khet" and "Changwat", what does that mean & what am i supposed to fill in there, if anything?

4] What does that say inside the photo space? I assume it is in Thai & says more in Thai than the english word "photograph" there.

I see the size of the rectangular photo space on my application form is about 3.5 X 5 cm.

Do they prefer the photo is glued, stapled, taped or paper clipped to the form? Or just loose?

I assume the reference to "arrival card" in post # 2 was mistaken & should have said "departure card". When i arrived in Thailand immigration took my arrival card & left me with one that says "departure" on it.

The rectangle size is a formatting error when printed out. The writing says 4cm X 6cm which is a standard size photo here from a photo shop

You glue the photo on.

The departure part of the arrival card is what they want. The TM6 is a 2 part form.

In case anyone is wondering about the TM6, the airlines always give me two pages with TM6 on each of them when i'm coming into Thailand. One of the two pages says "arrival card" and the other says "departure card". Immigration always take & keep the "arrival card", leaving me with only one page, the "departure card". It is the "departure card" that immigration want a copy of for a 30 day extension after one has already arrived in the country. Not the "arrival card".

"You can obtain a 30 day extension at any immigration office. Fee baht 1900 Passport Photo x 1Tm .7 Form Photocopy of current permission to stay stamp and departure card from passport.Photocopy of picture page from passport. You can go anytime but usually around one week before current permission to stay expires works best. To obtain another 60 day tourist visa you must leave the kingdom and apply at a consulate like Vientiane. If my memory serves me well."
