Nonthaburi Immigration Address

Could someone please advise their address in English ( we have it in Thai) but someone I know needs it in English. It's for sending in the 90Day Report by post.

I'm sure that, if you were to write the address on the envelope in your neatest Thai handwriting, the postman would be eternally grateful that, for once, he would not need to decipher a Thai address written in Roman script!
