You will need a completed TM7 application form with attached 4cm X 6cm photo, copies of your passport photo page, entry/permit to stay stamp and TM6 departure card. Plus 1900 baht for the fiee.

You will get the extension unless you are from a country that has a bilateral agreement for the 30 day entry.
