I've seen this also called the 'form for retirement income'. I need this form to complete my application for the retirement visa.

Suggestion: call the embassy/consulate of the country of which you are a national.

Help! I don't want to have to travel to BKK or Chiang Mai just to pick up this one lousy form!

I'm in Udon Thani.

As far as I know you will have to go to the embassy/consulate in person anyway to get the letter because it has to be endorsed (stamped/signed) by the embassy and to verify you are who you say you are. I believe it can be done on the spot, that is the same day.

Or do as Maestro said, call or e-mail the embassy to verify what you need.

I've already emailed the US Embassy in Laos which is only a 30 minute drive for me. They said they could notorize the form for me but cannot provide it. I went to Thai Immigration in Udon Thani where I live and they said they would accept the form notorized by the US Embassy in Laos. All the other items needed for the retirement visa I have and Immigration in Udon Thani can process for me.

So, I'm only lacking this one form. I really can't see having to travel all the way to BKK or CM just to get a blank form!

I will take the suggestion and email the US Embassy in BKK and see if they can mail/email this form to me but I doubt it because it's not posted as a download on their site.

I think that you want the Thai consul/embassy. Google search for Thai embassy and consuls in the USA.I believe that there is a form link on one of the websites, Wash. DC.,Chicago or Los Angeles.

I've emailed a friend in BKK and Monday he's going by the Embassy to pick up the form for me. If I can get the retirement visa going this route I'll post the form and steps I went thru.

Can't believe with all the retirees here in Issan no one has complained/ranted about having to travel all the way to BKK or CM just for one piece of paper. :o

Here’s and idea: once you get the form and before filling it out, scan it and post it here for other people who are in the same situation as you to print and use.

The US consulate in Vientiane probably does not have this form because it is not required by the immigration authorities of Laos and it never occurred to them that they might ever be called upon to provide this service to a US citizen living in Thailand. Good of them, though, to have agreed to notarise the document once you can get your hands on the form.

I suspect this document is on Embassy letterhead (don't have a copy to check) so it is probably best to copy the text indicating Vientiane Embassy rather than just use a copy - but that will depend on the wording used I suspect. If it says it has been sworn before an Embassy officer in Bangkok do not believe Vientiane will be signing it without a change.

Maestro, yes you are correct; no reason for the US Embassy in Laos to have a form pertaining to Thailand.

I'm going to post the blank form (if I ever get one :o ) only AFTER mine has been notorized by the US Embassy in Laos and actually accepted by Thai Immigration. As we all know sometimes things are said/promised and then later 'changes' occur. I want to make sure this really works before I post something misleading.

Is this "form" a new idea? For my retirement extension I obtained a letter (which may have been a standard format) from the British Embassy in Bangkok in 2000. I take two copies of the letter to immigration every year but retain the original.

For information the letter reads:

Addressed to Commissioner, Immigration Bureau.

Dear Sir,

Mr _______________________

This is to certify that Mr __________________, holder of British Passport No ___________ issued UKPA on ______________, is recognised as a British Citizen.

Mr _______________________ has indicated that he resides at ___________________________ and there is no information in our Consular records to the detriment of the applicant.

Mr _______________________ has provided evidence showing that he receives a monthly pension of ___________.

The Embassy would be grateful for any assistance you can provide to Mr _________________, in his application for an extension to stay in the Kingdom.

This is the US Embassy and they have a set form that you fill in your name/income and then sign. Believe people have been able to use over is they make copies but I have just obtained new each time (living in the big city).

Thanks for posting this. I wish someone could post the form/letter used by the US Embassy. I suspect it is very similar; I hope so.

I have emailed the American Citizens Service and hope to get a reply on Monday. I also have a friend (Jason Bourne) :o who will go by the US Embassy in BKK on Monday to get the form for me.

Unless I missed it the form is not available as a download on the US Embassy web site. If it was it could save people time and speed up the process at the Embassy; a person could have the form already filled out and just walk in......pay the fee.....done.

Believe they issue a control number when they give you the form and a bill to present to the cashier. After payment you get receipt and present this with your passport/form to US Consular officer and when called you report to window to sign in his presence.
