my extension has to be renewed by the 5th of March, but I intend to travel towards the end of January. My question is; do I have to be back by the 5th March or can I renew the extension at a later date?

You must have a new extension and re-entry permit before travel or return before March 5 using current extension/re-entry permit - but there is nothing preventing starting again with a new non immigrant O visa entry for your return and then extend that. You could show immigration your travel plans and ask if they can do extension early.

Some offices will let you apply 45 days early.

They might even let you do it earlier if you explain.

If not you can start again with a Non Imm O Visa.
